About Chin Augmentation

Chin augmentation can either be a surgical or non-surgical procedure that increases the projection of the chin. Non-surgical options include fillers, but Dr. Stanislaw creates safer, better, and longer-lasting results by surgically placing an implant. Candidates

Both men and women who have “weak” chins can benefit from a chin augmentation. Often, patients with weak, recessed, or small chins perceive their nose as too large for their face. These patients present for a rhinoplasty consultation to reduce the size of their noses, but in reality, they may need a chin enhancement to achieve facial balance instead.

Another common concern in patients with weak chins is the illusion of excess fat below the chin. Often, it is the poor bone structure of the jaw and neck or the receding chin that gives the appearance of excess fat. In this situation, chin augmentation corrects chin structure and adds extra mass, causing the skin and fat on the neck to stretch and become slimmer. The final indication for chin augmentation is when you are experiencing age-related bone resorption (osteoporosis) of the jawbone directly in front of the jowls. The bone loss makes the jowls look worse. In this case, a chin implant can augment the pre-jowl depressions and hide the jowls, delivering a more refined, straight jawline.

Personal Consultation

Your consultation is your opportunity to explore what you can achieve with chin augmentation. Here, Dr. Stanislaw will first review your medical history, and together, you will discuss your goals and achievable outcomes. Dr. Stanislaw will measure your chin by making two lines, one from the suborbital rim under the eye going toward the ear horizontally, parallel to the floor (called the Frankfort Plane), and another perpendicular from the nasion (top of the nose) or maxilla crest and down. Your chin should meet this perpendicular line. If it falls short, it is hypoplasia of the pogonion (a weak, recessed chin). This diagnostic process will inform your treatment options. Following this, you will try on chin implant sizers. These devices will help determine the ideal shape, placement, and size of your implant. With this information, Dr. Stanislaw will tailor a treatment plan to your case. For certain patients, he may also recommend additional procedures to enhance final outcomes. Preparation

Dr. Stanislaw will provide detailed preparation instructions after you and he finalize your treatment plan. Following these closely lays a foundation for a safe, successful procedure, smooth recovery, and optimal results. These may vary for each patient depending on their concerns and medical history, but common guidelines include:

  • Ceasing or limiting nicotine use before and after treatment
  • Discontinuing blood-thinning medications and supplements
  • Filling prescriptions you may need during your recovery
  • Making the necessary arrangements away from work and other responsibilities
  • Avoiding alcohol consumption before and after your procedure
  • Arriving to your appointment with a clean face free of moisturizers, makeup, or sunscreen

Procedure

Dr. Stanislaw performs chin augmentation on an outpatient basis with most patients receiving local anesthesia only. General anesthesia and/or sedation, however, are available for patients who request it. Once you are comfortable, he will make a small incision at the underside of the chin. This under-chin approach is favored, as it avoids exposure to oral bacteria. Once he makes the incision, he will create a precise pocket in front of the chin bone and insert the implant, working precisely and carefully to avoid nerves. Dr. Stanislaw specifically uses a Goretex extended anatomic chin implant for the advantages it holds over other options (such as silicone/plastic, ceramic, or button-shaped implants). If necessary, Dr. Stanislaw can shape the implant with a scalpel, making minor adjustments to match your anatomy. These implants concentrate fullness in the center, but they have “wings” that branch out on either side. These help add a natural-feeling and natural-looking fullness into the prejowl depressions, eliminating the risk of a “witch’s chin” appearance. The center mass of the implant will sit above the periosteum in the center of the chin (supraperiosteal midline placement), while the wings will sit beneath tissues on the sides (subperiosteal placement). Because Goretex is porous, tissues grow into the implant and mesh it into place, eliminating the need for screws. Instead, once the implant is in the optimal position, Dr. Stanislaw will place a suture to keep it in place as tissues heal. Following this, Dr. Stanislaw will close the chin incision with techniques that minimize scarring, apply dressings, and allow you to briefly rest before returning home. The entire procedure typically takes between 90 to 120 minutes.

Recovery

Dr. Stanislaw will provide you with detailed recovery instructions after your procedure. On average, patients face a week to 10 days of downtime. (2) Throughout this time, you will take antibiotics to fight infection, take celecoxib (a non-opioid, non-blood thinning pain medication that will also help reduce swelling), and keep your stitches covered with petroleum jelly (Vaseline/Aquaphor). Chin augmentation does not require any diet restrictions (such as a limit to soft foods), but you will need to avoid bending over and follow certain other activity restrictions. Throughout your recovery, you will need to rest and elevate your head to promote circulation.

Swelling will be prevalent, although Dr. Stanislaw’s patients rarely see significant bruising. However, the chin will first appear too big due to swelling, but it will reduce to a proportionate, ideal size as this dissipates. After one week, you may start to return to light work, but you must avoid heavy lifting and intense exercise. Dr. Stanislaw will remove your sutures at a one-week follow-up appointment, and you will return for two more follow-ups at the one- and three-month marks. As your tissues settle and swelling dissipates, you will begin to see final results emerging.

Results

Your chin augmentation results will manifest as a stronger and more balanced profile with improved facial harmony. These enhancements can also create a slimmer neck and a sharper, more defined jawline. By correcting a weak or receding chin, some patients also bring the nose and forehead into harmony with other features, including their new chin.

Potential Risks

The risks associated with any facial surgery include bleeding, infection, and scarring. Additional risks to chin augmentation include, but are not limited to, asymmetry and abnormal contours of the chin due to buckling or bunching up of the implant, temporary or permanent numbness of the lower lip, injury to the nerve that moves the lower lip, overcorrection of the chin augmentation, and a sub-optimal aesthetic result. (3) However, under the care of a dedicated, experienced expert such as Dr. Stanislaw, the likelihood of these risks remains very low.

For some patients, chin augmentation can be an effective alternative to a face and neck lift. For others, chin augmentation enhances the results of a face and neck lift.
